  • Incumbent is a file clerk for the Health Information Management (HIMS) section of the Health Administration Services (HAS) service line
  • The HIMS Scanning Unit incumbent assists the medical center in the transformation of paper documents to electronic documents in the Computerized Patient Record System (CPRS) via Vista Imaging.
  • The incumbent also aids in the form of scanning and uploading of documents for medical center programs and services.
  • The incumbents’ primary duties of this position are to maintain accessible records and provide efficient service for direct and indirect patient care by managing the receipt, uploading, or scanning, and indexing of health documents in a timely and accurate manner.
  • The incumbent is responsible for carrying out a variety of health information management functions.
  • The incumbent performs duties related to the receipt, intake, scanning, uploading, indexing, quality control, destruction, and transfer of health and administrative information.
  • The incumbent is responsible for electronically scanning or uploading various health information documents into Vista Imaging and assuring that the scanned documents are associated to the correct patient, progress note, consultation, or document type in the Computerized Patient Record System CPRS.
  • The incumbent must follow scanning and uploading procedures established by the HIMS/HAS service line.
  • After completing the scanning, uploading, and indexing process, confirms record integrity, and reviews the image quality in the Vista Imaging display.
  • Ensures regular quality checks are completed per prescribed timeframes and criteria. The incumbent notifies the supervisor when images are scanned or uploaded into the incorrect patient’s record, program area, documents are indexed incorrectly, or the image is corrupted.
  • Notifies the supervisor of the need to rescind Advance Directives in CPRS as specified in Handbook 1004.02, Advance Care Planning and Management of Advance Directives.
  • The incumbent determines which documents may exist based on whatever information the requester has supplied and on the history of the agency’s record keeping procedures for the time period involved.
  • When documents are received, the incumbent reviews each page for appropriate scanning criteria and ensures that all necessary health or administrative information meeting the criteria is integrated into CPRS through the scanning or uploading software and hardware.
  • The incumbent ensures prior to scanning that all paper clips, staples, tape, and loose or hanging tape are removed from the documents.
  • The incumbent scans filing or document materials by indexing to the appropriate clinical or administrative area in Vista Imaging to facilitate future reference.
  • The incumbent also correctly indexes all documents imported or uploaded from other devices or sources, including pdf, tiff, jpeg files, etc. into Vista Imaging. When all efforts to locate within central files are unsuccessful, searches through appropriate office files, contacts subject-matter specialists in order to determine whereabouts of required file materials.
  • Incumbent maintains confidentiality of all electronic records and paper components of the health record or medical center program materials; follows appropriate procedures when mailing or destroying health information in paper or electronic format; and sensitive (employee and designated patient records) health records are maintained in a secure manner and available only to requesters in the scope of their official duties.
  • Utilization of a computer to perform a wide variety of record keeping, correspondence, and tracking operations.
  • The incumbent performs timely filing, retrieval, and clerical duties associated with the retention, maintenance, disposition, guardianship, and control of records in a variety of filing systems to include but not limited to terminal digit, alphabetic, numerical, or date order.
  • The incumbent is responsible for communicating or contacting administrative and clinical staff members to locate/obtain health records, documents, or requested information.
  • Introduces and explains documents received from community care providers, medical centers, or organizations to medical center staff and assists in understanding and recognizing the nomenclature and indexing terms used to allow access the information efficiently.
