Conduct Psychosocial Assessments to include BIMS and PHQ2-9 both quarterly and annually, to identify individual needs and specific social services necessary to help in addressing each resident's objectives, needs, and goals.
Build and maintain strong interpersonal relationships with residents, and to help resolve issues by responding in a timely manner.
Educate residents on various resources, and programs that are available to them.
Set up home health referrals and other services that residents may need upon discharge back into the community.
Provide services as needed for visiting ancillary services for the residents within the facility.
Grievance Official and Advocate for all residents and their rights and following up regarding any concerns they may have.
Schedule weekly Care Plan meetings with residents and families.
